The Joy of Sex

Well the book “The Joy of Sex” is being resurrected and there was mention of it on Night Line. Yes that is the manual that was very popular in the 60’s and 70’s. However, even though it talked about positions, and areas of the body that are sensitive and never touched on how to make them sensitive in case they were not. How to make errogenous areas more errogenous. Well, that is where labiaplasty and vaginoplasty come in. It is hard for the sensitive areas in our body to become sensitive during sex when they are covered with lots of skin or when the area is so stretched that the sensitive areas do not get touched. It is true that different people have different “sensitive” areas, like the ears, the belly button, the neck etc. It is pretty much agreed that there are some areas in women which are always sensitive if given a chance. That is the clitoris and outer labia minora and the anterior part of the lower vagina. Thus very large labia or clitoral good cover the clitoris and the lower part of the outside of the labia. Therefore they become less sensitive. Labiaplasty by making the labia smaller enhances the sensitivity.Reduction of the clitoral hood also allows for better sensitivity. It is similar to uncovering the glans of the penus by retracting the prepuce or excising it altogether at birth or during certain religious ceremonies-Judaim after birth and moslem at age 13yo. As far as the anterior part of the vagina or the G spot -that becomes more sensitive after the vagina canal is tightened as in vaginaplasty. With a smaller canal that spot gets a lot more stimulation. Of course the G spot injection is doing similar thing by plumping the area up. If one does both vaginaplasty and G spot injection the sensitivity is phenomenal. These procedures are important especially after multiple childbirths and after gain and loss of a lot of weight. Some people are born with large labia and that only gets worse with childbirth, weight gain and menopause. Thus the joy of sex cannot be complete unless all those areas are tended to if needed. Both procedures are short and safe provided done by an experienced physician. Thus the previous generation learned about how to enhance what women have by learning about where the areas are and how to get to them, but today we can do it by surgery as well so that once these areas are reached they respond.

Does the Sex of the Surgeon Matter?

LAST June, after Jerri Hafizi, a 5-foot-3 construction firm executive, lost 65 pounds, she asked a plastic surgeon to tighten the loose skin around her torso and lift her breasts. But the surgeon seemed not to understand what she wanted. He squeezed Ms. Hafizi’s skin in his fists, she said, and told her that the only way to solve her problem was with implants.

“He wanted to make me into a Barbie doll,” said Ms. Hafizi, 50, who lives in Merritt Island, Fla. “But I didn’t want to be made over according to somebody else’s idea of what a woman should look like.”

Overseas Plastic Surgeon: Two Horror Stories.

Late last year, a French surgeon who practiced–illegally–as a plastic surgeon was sent to prison for four years. Before being caught, Michael Maure had performed botched operations on literally dozens of women. Maure was sentenced and now awaits extradition from Spain, where he traveled soon after the scandal broke.

As is usually the case with cosmetic-quacks, Maure did not have proper credentials for performing plastic surgery. He attracted clients by offering unusually cheap rates at a substandard clinic. Many of the women who had procedures by him reported leaving the clinic either permanently disfigured and / or in considerable pain.

Each of the women testified at his trial. Though Maure was absent from his own trial, the court found him guilty of illegal practice, wounding, and endangering life. He not only will have to serve a prison sentence, but he was ordered to compensate his victims and to pay a fine of €75,000 .

Meanwhile, and almost at the same time, another quack surgeon was also destroying people’s lives in the West. In January, Costa Rican authorities picked up a cosmetic surgeon named Dean Faiello, who had been suspected in the death of a Manhattan man who has been missing for some time. The man had been a patient of the doctor’s, and police almost immediately suspected that the surgeon had killed him accidentally in a botched operation.. Police found Dr. Faiello drinking a cocktail at a tropical resort, by the pool, according to authorities. As soon as Faiello saw the police, he raised his hands and shouted, “No problem, no problem!”

The moral of this story is not that all overseas cosmetic surgeons are incompetent. In fact, there are quite a few excellent ones. However, remember that the United States has stringent credentialing requirements that are not always required in other countries. So if you travel overseas for a labiaplasty, vaginaplasty, or any other cosmetic procedures, ask credible authorities in the States if they know of the doctor, or if they can help you to verify his/her credentials.

Ask whom, ask what, ask when

There is a lot of confusion as to what one can ask a doctor comfortably about things other than health without looking inappropriate and foolish and wasting time. It may surprise you, that a good doctor is interested or should be interested in your entire well being and that of your family. So if you have a question about anything pertaining to your body you should ask and if he/she does not know the answer, she/he can refer you. One such question that always comes up is discussing the cosmetic look of the perineal or genital region. There is a great amount of change that occurs in that area from childhood to old age. Some changes are due to age and added to that is the changes brought about by weight fluctuation, and child birth. Some women also develop various conditions during their lifetime, i.e. Bartholin infections, urinary track infections, herpes, fungal infections, stress incontinence, urinary frequency, constipation, hemmorrhoids and many others which contribute to the changes of the perineum. Menopause and some cancerous lesions also contribute big time to the changes. So, no wonder that the genital area changes drastically and also needs maintenance. However, since most women do not like to look at themselves, they are not sure what changes occurred, are they normal and is there something to do about it if they do not like the look of it. However, most feel a bit embarrassed to be preoccupied with the appearance of the genitalia especially since the appearance has nothing to do with body health, just “vanity”.  Well many things we take care of are “vanity” things. One should be just as comfortable taking care of their genitalia. Actually examination is important because that area is also subject to tumors and one should examine the area to signs of changes. For example dark discoloration could be a sign of melanoma and small skin cancers can occur there too. It is part of the body and therefore one must be familiar with that area no matter how we feel about it. So ask about the appearance, or any lumps or bumps or lesions. Cosmetically one can reduce the labia minora, remove the dark pigmentation of the labia, unhood the clitoris if too covered and interfering with sensation, one can raise the mons pubis, and one can reduce the labia majora as well and even plump them up. One can tighten the vagina entrance by repairing loose muscles and one can even have a hymen reconstructed. A bladder can be lifted with a small band to stop stress incontinence and hemorrhoidal skin tags can be removed. So do not worry about asking whatever insignificant a question you think you have because it is good to know.

Vaginal Lubrication

Vaginal Lubrication

The purpose of vaginal lubrication is to allow comfortable entry and thrusting of the penis during intercourse. Being  sexually “turned on”, even without the vagina being touched, will result in an increase in vaginal lubrication. The factors at work here are (1) psychological receptivity, (2) a normal level of estrogen that maintains the vaginal lining appropriately thickened so that there is a constant normal shedding of vaginal cells from the vaginal surface that provides a lack of dryness.

There are small glands (Bartholin and Skene’s)  that have been identified that surround the vaginal opening. It was once believed  that these glands provide the lubrication necessary for sexual intercourse. It is now very clearly established that the vagina  “sweats” from the entire surface. The outer glands, which sometimes may become infected and may swell to the size of a golf ball before draining, play an insignificant role in vaginal lubrication.

In older women who may express symptoms of having increasing vaginal dryness, estrogen may be applied with cream, a small retained plastic ring that releases a small amount of estrogen over a 3 month period or a small estrogen pellet  that  dissolves over 12 to 24 hours and may be inserted twice weekly. For milder forms of dryness, or in women who have specific reason not to receive estrogen, we have samples in the office of excellent lubricants and also there are commercial  over the counter lubricants that one can buy. Some of these products a pleasant odor and may be administered prior to sex. Their principle function is to reduce friction.

Urinary Incontinence

Urinary incontinence,  or involuntary leakage of urine is a common problem, but one not discussed freely in a doctor’s office. The onset of this condition may occur in one of two ways. Following a traumatic vaginal birth there may be a very sudden recognition by  the new mother that she is unable to control the flow of urine. The onset comes very shortly after delivery and may occur with a variety of movements and activities by the mother. In a majority of cases urinary control returns within a few weeks, and sometimes within a few months. Muscle exercises are taught and control returns, though perhaps not with the same degree of confidence as before.

More commonly, and eventually of greater concern, is the leakage of urine that happens with coughing, sneezing, changing position and while on a tennis court or jogging. This type of incontinence begins often around the time of menopause, as the menstrual periods cease. The original loss of urine may be very slight, and noticeable only with certain physical activities.

Its slow onset may be tolerated with external napkins and may not seem necessary to bring to the doctors attention. In fact, the doctor may not regard the early forms of this problem worthy of discussion. That is unfortunate, because a woman should learn to look out for its progress and be able to determine specifically under what circumstances the leak will occur. Muscle exercises, called Kegel muscle exercises are often very helpful in an early stage of the condition. One is instructed to make believe one is urinating, and wants to stop the flow in the middle of the  activity. This is a simple form of exercise that can be done by the patient at her own pace, throughout the day. The effectiveness of the exercise should be tested by the doctor with a simple vaginal exam. If the patient is instructed to stop the flow of urine, then the levator muscles will tense and become firm. Both the patient and the doctor are aware of the levator muscle response. In some instants  it may be advisable to be  taught to do a Kegel maneuver by a physical therapist.

Vaginal dryness and thinning of the vaginal lining may promote loss of urine and this condition may be reversed by a prescription for vaginal estrogen, to be applied once or twice weekly.

Urinary incontinence, when accompanied by frequency of urination may be due to a urinary tract infection (UTI). Providing a sample of urine in a doctor’s office or at a local laboratory may provide a diagnosis and the appropriate antibiotic necessary to correct the condition.

With advancing years, as our muscles weaken, those muscles holding the bladder in place and controlling the outflow of urine may need repair. In recent years this has become a relatively non-invasive procedure, capable of being performed in an ambulatory setting, without an overnight stay in the hospital.

Surgical repair of urinary incontinence may be done as a single operation, though it may be accompanied by other operations in the pelvis  to lift some of the body parts or improve the cosmetic appearance of the labia or vagina.

Currently, one of the most effective procedures is to place a thin, permanent strip of mesh underneath the bladder, near the point where the urethra (the muscle tube that leads from the bladder to the outside) attaches to the bladder. The mesh acts as a backstop to prevent the bladder and urethra from descening more than a normal distance, thereby controlling the flow. Most women do not require a catheter after the procedure and within 1 days are able to conduct normal activities.  Sexual intercourse should be avoided for 4- 6 weeks.

Ask What, Ask When, Ask Why

Women go for doctor examination yearly. They undergo pap smears, mammography, and internal examination and if no pathology is found they are told all is fine and come back next year. However, often women want to ask about some “insignificant things” but do not, as they feel that there is no time, the concerns might be silly, insignificant and not worthy to discuss.  Often they do not know even what to ask or how to ask. It is true that often doctors are very busy as they are assigned many patients by the insurance companies per day and do not have time to talk about things that do not threaten a life. I,therefore, think that this blog and other blogs on women issues will serve to answer many concerns.  Some of the problems that women face is urinary frequency due to chemical irritation rather than a real infection. These problems can become chronic and terrible disconcerting. The frequency can lead to constant wiping and irritation causing hypertrophy of the labia which in turn cause problems in wearing clothes and hygiene. These issues then may lead to depression and irritability. These women often cannot go on hikes, or ride on a bus or spend time in places far from a toilet. Social life is very hard and coughing or sneezing embarrasing. This urinary frequency has to be investigated and indeed if it is not an infection, botox or other medications can be used to control it. Sometimes a band can be inserted surgically to control stress incontinence. The labia can then be trimmed to normal size and a normal life can be followed. Psychological and stress relief is a welcomed result of these treatments. I think women should talk with each other and find out about all the conditions that each may be worried about and then maybe they will learn how to treat them and whom to see. No one needs to be alone with their questions as someone out there knows the answers and may help if one asks.

Sexy Clothes for sexy bodies

Summer will be here soon and again short skirts, bathing suits and shorts will be worn. As we come out of hybernation we look in the mirror and often would like to tweak certain parts to make them look good and attractive. Liposuction becomes very important. However, did you know that you can do liposuction of the body and preserve the fat and then inject it wherever you need it. Often people inject fat into labia majora to make them look more youthful. Sometimes the pubis gets suctioned and that fat used for the labia majora. Tired hangy labia majora can become rejuvenated if they are plumped up just like the facial wrinkles and depressions. It is especially important when one waxes or shaves or lasers the mons pubis hair. Laser is a much more permanent way of removing hair than waxing, shaving or using any pastes like nair. Nothing is more gratifying than making the stomach flatter, and the buttock tighter and the labia plumper. It all looks great for the summer. Think about hair removal as well. The pubic hair removal is very popular with laser if indeed you are serious about removing all that unwanted pubic hair.

Vaginal dryness

Very often after menopause or if one had tx for Breast Cancer with chemotherapy vaginal dryness occurs. It is very good to use lubricants because that reduces pain on intercourse and also reduces irritation of the whole vaginal area. The Colen MD office has a very nice lubricant sample that you may request to try. It is just the right consistency and feel and smell and even taste. For women who exercise by bicycling or spinning vaginal dryness may cause bleeding and pain. So consider lubrication. After all we lubricate our skin and especially our faces at night. What is so unusual about lubricating the vaginal area? It is prone to yeast infections, Barthelin gland infections, genital warts, various allergic reactions and rashes. It must be taken care of as any other part of our body. So ladies pay attention and take care of every part of your body and that includes the vaginal area as well.
