Confidence is the Key: Surgery in the Pelvic area

For many women, sexual confidence is difficult to achieve. As women become more independent and equal with men in education and in earning a living bashfulness and innocence is also changing. More women talk more freely about the appearance of their pelvic structures just as the men do. With that freedom even magazines and shows discuss the “accepted” and “desired” look of a woman’s labia and vaginal area. Magazines, celebrities, and the media create an image of what the “ideal” woman’s genitalia should look like. It has become very popular to have bikini waxes and to trim all or some of the pubic hair, as well as wearing more revealing lingerie, clothing and swimwear. Because of this focus, more and more women ask for plastic surgery to fix what they may consider abnormal or unattractive looking labia or vaginal area.

Libido in the postpartum

Many religions teach that sexual relations should only occur for the purpose of propagation. Therefore, contraception and the pleasure principle should not be considered. It is interesting to note that in a way biological insticts dicatate same but only after the female gives birth. Many women complain of loss of libido after giving birth. That of course lasts for up to one year for some, a lifetime for others.  Some women not only have a loss of libido but become quite depressed and, therefore, interest in sex is far down on their list. Psychologists try to treat that phenomenon and some women are put on antidepressants, because that attitude puts a tremendous stress on their marriage. That is perhaps nature’s way of telling the female that now you had a child and you did your part in propagation of the species. Now it is time to rest and nurture this child to ensure its survival before you go on to have more children. Nature, protects the offspring to make sure the species continues. When the child is healthy and growing, often libido returns and more children are born, or not depending on the fertility of the female. Loss of libido after childbirth therefore, is not pathological and should not be of concern. Biology, restores it when the time is right. The worse thing is if one does not understands this phenomenon, and then anxiety sets in which then leads to further deterioration of the natural development of the natural biological instincts which is attraction to the male- libido, and the pleasure principle which is necessary for the propagation of the species and was set by the natural forces of nature.

Variety of methods in Labia minora reductions

Many women request reduction of the labia minora for functional, aesthetic and social reasons. The demand for this procedure is not only in the US but  all over the world. Labia minora hypertrophy can be congenital or acquired by chronic irritation, exogenous androgenic hormones or constant stretching by mechanical means. Large labia minora can cause inflamation, poor hygiene, interference with sexual intercourse and also with many athletic activities. Today many women are more aware of their own genital appearance because of pattern of shaving and the availability of published nude pictures in magazines, movies, videos and on the internet. Abnormally large labia  lead to self-consciousness sexually and low self esteem. Some worry that some may judge the person as engaging in masturbation. However, in women urinary frequency is quite common and the constant mechanical stretching due to wiping could cause some hypertrophy indeed. More on frequency later. Here would like to discuss that there are at least three methods of reduction. One mainly removes the extra tissue at the edge of the labia leaving the same shape, but thus getting rid of a lot of the pigment at the edge. Some women like that as they like to have the pink color to the labia minora. Other methods involve wedge resections, however it may lead to a shortened labia with some tightness. Another method is removing excess labia mucosa from the midsection lenghtwise which preserves the edge and the pigment. Whichever method one chooses, if done by a competent surgeon has few complications and a short recovery and can be done on outpatient basis under sedation anesthesia. However, one must discuss the various methods with the doctor and choose the one best suited to the outcome the patient would like. Restoring Virginity

In many religions and cultures it is very important that women are virgins when they marry. The common symbol of virginity, the hymen (a thin membrane located at the entry of the vagina), by tradition should break on intercourse causing the new bride pain and  bleeding. Blood on the sheets in some countries and traditions is then shown the family to reaffirm that the bride was a virgin and now she consumated her marriage. (more…)

Intercourse after giving birth

For many couples, having a baby is both rewarding and stressful.  During delivery there could be tearing of the vaginal mucosa which is quite painful.  That is the physical pain. However, some women continue to have emotional pain after the delivery. That emotional stress and pain may occur from a decrease in the quality of their sex lives and their own diminished libido. Some women are very upset about the change in their body following the pregnancy and do not feel attractive anymore.  Although the vagina shrinks in diameter after giving birth, the vagina never truly returns to the diameter that it was before giving birth. Even the use of kegal exercises rarely makes much of a difference and the vagina stays at its larger diameter, which can make sex less satisfying as the woman and her partner do not feel the same contact.  Common complaints are low self-esteem, difficulty achieving orgasm, and a general feeling that it doesn’t feel as close of a “fit” for both woman and partner as before. To make intercourse better for both partners one must consider having vaginaplasty.  Vaginaplasty which is also called vaginal rejuvenation,  eliminates unnecessary excess vaginal lining and tightens the muscles compromising  the vaginal walls  that become loose or weak during birth. This results in a smaller vaginal canal. The smaller vaginal diameter creates the ability to contract and control muscles easier making a closer fit between partners during sex. This creates more resistance and an increase in sexual pleasure for both partners. Women tend to climax easier because the procedure increases sensitivity to the vagina. The procedure  is performed on an outpatient basis. Recovery lasts a few weeks and within 6-8 weeks intercourse may initiate. Following a vaginaplasty the vagina will be closer to the diameter that it was previous to conceiving. Female Sexuality

Why is everyone concerned now about labiaplasty? I think it has to do with all the laser hair removal, bikini and brazilian waxing fad and of course the more revealing clothing. The talk shows and reality shows also bring forth attention to the female genitalia. For a long time the male genitalia was the buzz. Length, width, erection problems, impotence esp due to diabites, or low blood supply. No one bothered with the females. Well Viagra took care of the males and now we must take care of the females. Mons Pubis Reduction

A 30 year old woman comes in complaining of bulging of the mons especially when she wears pants. She gained a lot of weight at one time and now she lost it everywhere except around the mons pubis. What can be done? Well in her case, since she is young with elastic skin, suction of the mons pubis will be quite successful. Of course, if she waits, till she is older, and the skin stretches more and loses its elasticity, a skin excision will also need to be done, either through a small incision at the border of the pubic hair, or during a tummytuck if one needs one. Either way, these unwanted bulges can be fixed. It is always better to do it at a younger age but of course at any age help is available. Role of Estrogen and Progesterone

While estrogen is produced by the ovaries, it mainly acts on the vagina, uterus, breasts, bone, skin and the female distribution of fatty deposits in the body. After menses cease, around age 51 or 52, in what we call the menopause, there are subtle changes that gradually become more pronounced. Without estrogen replacement the vagina begins to shrink and the normal rippled contour of the vaginal wall becomes thinner, straightened out, and more sensitive to touch. Taking estrogen reverses these changes, and allows the woman’s vagina to produce its own lubricating secretion.  If estrogen is started soon after the occurrence of menopause the vagina may never become thin, narrow and sensitive. Progesterone has no value for the vagina. It is required to be taken by women who receive hormone replacement therapy during  menopause to prevent the lining of the uterus from becoming too thick, and potentially cancerous. written by Dr. Robert Porges
